Membership Fee Inclusions
Your membership fee covers the core services of your primary care relationship, including:
A comprehensive annual visit with individualized care planning
Office visits designed to allow adequate time for discussion and evaluation
Same-day or next-day scheduling when clinically appropriate
Direct communication with your physician through secure messaging
Coordination of care with specialists and outside providers when needed
Ongoing preventive care and chronic condition management
Membership fees do not include laboratory testing, imaging, medications, or specialist fees, which are billed separately by outside providers.
