Rather than working within the constraints of volume based care, this approach allows for thoughtful visits, reliable access, and ongoing physician involvement. It supports care that is steady and continuous, not reactive or episodic.
For patients, this means fewer handoffs, clearer follow-through, and a physician who remains closely involved in decisions over time.

Membership Fees
StoneBridge Medicine makes high-quality care accessible at a predictable monthly investment.
There is no long-term contract required. While concierge medicine works best as an ongoing relationship, you are not locked into a fixed term.
Details regarding cancellation, re-enrollment, and membership terms are outlined in the membership agreement provided prior to enrollment.
Membership Fee Inclusions
Your membership fee supports a more personalized, accessible, and coordinated care experience, including:
A smaller patient panel designed to allow more time, familiarity, and continuity
Unhurried visits and extended appointment availability when clinically appropriate
Same-day or next-day scheduling when available and medically appropriate
Direct communication with your physician through secure messaging
Coordination with specialists, hospitals, rehabilitation facilities, home care agencies, and outside providers
Review and organization of outside records, test results, and care plans
Longitudinal health planning, preventive guidance, and support between visits
Administrative assistance with care navigation, forms, referrals, and follow-up planning when appropriate
The membership fee is not insurance and does not replace Medicare or commercial health insurance.
For Medicare beneficiaries, Medicare-covered services are billed to Medicare as required.
For commercially insured patients, StoneBridge Medicine is out-of-network and does not routinely submit claims to commercial insurance plans. Documentation may be provided upon request for possible out-of-network reimbursement, but reimbursement is not guaranteed.
Laboratory testing, imaging, medications, vaccines, specialist services, hospital care, emergency care, and services from outside organizations are not included in the membership fee and are billed separately by those providers or facilities.
