Understanding Concierge Medicine

Concierge medicine is a membership-based model that allows for more time, access, and personalized care. By limiting the number of patients in the practice, I am able to offer longer visits, same or next day appointments, and direct communication without the rushed experience common in traditional practices.

Our practice is devoted to the maintenance of your good health and proactive healthcare.
Our goal is to limit or eliminate your need for urgent care clinics by being responsive to your concerns early. We are also available via phone, text, or virtual visits when you are away from home.

StoneBridge Medicine maintains a limited patient panel to ensure responsiveness. Once the panel is full, a waitlist may be maintained.

Prospective patients are invited to request a brief introductory call through the website to determine if the practice is a good mutual fit.

We offer home visits for an extra cost. Please book a consultation to find out more.

We are a primary care clinic. There may be conditions or situations that are better served by a specialist. We will work hard to facilitate that connection for you.
Membership, Fees, and Insurance

Your membership supports a more personalized and accessible care experience, including a smaller patient panel, unhurried visits, enhanced appointment availability when clinically appropriate, direct non-urgent messaging, care coordination, review of outside records, and help navigating referrals, testing, specialists, and follow-up planning.
The membership fee is not insurance and does not replace Medicare or commercial health insurance. Medicare-covered services are billed to Medicare as required. StoneBridge Medicine is out-of-network with commercial insurance plans and does not routinely submit commercial insurance claims.
Labs, imaging, medications, vaccines, specialists, hospital care, emergency care, and outside services are not included and are billed separately by those providers or facilities.

Membership fees are billed monthly. Accepted payment methods are outlined during enrollment and detailed in the membership agreement.

StoneBridge Medicine is a membership-based practice. Membership fees are paid directly by the patient and are not billed to insurance.
For Medicare beneficiaries, Medicare-covered services are billed to Medicare as required.
For patients with commercial insurance, StoneBridge Medicine is out-of-network and does not routinely submit commercial insurance claims. Documentation may be provided upon request for possible out-of-network reimbursement, but reimbursement is not guaranteed.
Patients may still use insurance for outside services such as labs, imaging, medications, specialist visits, hospital care, and other services not provided by StoneBridge Medicine.
This model allows care to be more personalized, accessible, and less constrained by traditional scheduling limitations.

Memberships are billed monthly and do not require a long-term contract. While there is no minimum commitment, the model works best as an ongoing relationship.

Details regarding cancellation, re-enrollment, and notice periods are outlined in your membership agreement. There are no long-term contracts.
Appointments and Access

Yes. Same-day or next-day visits are often available depending on clinical urgency.

No. Visits are scheduled to ensure adequate time and attention for each patient.

If something comes up, please let us know and we are happy to reschedule. There are no penalties for no-shows or cancellations.

Concierge medicine offers enhanced access, but it does not replace emergency services. Urgent issues are addressed promptly, and guidance is provided for after-hours concerns.

All emergencies should be handled via 911 or the ER. Members are provided several methods to contact our team for urgent or non-urgent matters.
Communication and Specialty Care

Members can communicate through secure messaging, phone calls, and scheduled virtual visits. Messages are personally reviewed and typically addressed within one business day.

Yes. I assist with specialist referrals, reviewing outside records, coordination of care plans, and interpretation of test results to reduce fragmented care.

I assist with referrals and communication with hospitals to maintain continuity of care when outside services are required.
