Office Hours
At Stonebridge Internal Medicine, our hours of operation are from 8:30 a.m. to 5:00 p.m. Monday through Friday.
New Patients
Please arrive at least 15 minutes prior to your appointment time to allow enough time for your information to be registered within our systems. Please bring the following items to your first visit:
- Current driver's license or state issued photo ID
- Current health insurance card(s)
- Completed new patient forms
- A list of your current medications
- All diagnostic tests ( MRI, X-rays, CT scan)
- Your pharmacy name and phone number
Late Arrivals
For the consideration of other patients, please be advised that if you arrive 15 minutes or more late from your scheduled appointment time, you will be asked to re-schedule.
Cancellations
We do attempt to call you 48 hours before your scheduled appointment as a reminder. However, if your contact numbers have changed since your last visit, we may be unable to contact you. The office requires a 24 hour cancellation notice prior to your appointment, if you are unable to notify the office, a cancellation fee will be assessed and charged to your account.
After Hours
If you need to contact the office for any emergency, we have a 24 hour answering service to dispatch your call. A personal operator will forward your name and number to the on-call physician who is available daily to handle your emergent calls.
Prescription Refills
You have several ways to refill your prescriptions. You may call your pharmacy and have them fax a refill request, or you may leave a detailed message with the nurse. We can also electronically transmit prescriptions to the pharmacy on your behalf if you can provide the fax number.
