Stonebridge Internal Medicine is now accepting new patients.  Minors 16 to 17 years of age must be accompanied by a parent or legal guardian in order to be treated by our physicians. All new patients should arrive 30 minutes prior to their appointment time.

• Current driver's license or state issued photo ID
• Current health insurance card(s)
• Completed new patient form (if applicable)
• All diagnostic tests ( MRI, X-rays, CT scan)
• A list of your current medications
• Co-payments
